For this purpose, the beneficiaries must present the following documents: medical prescription or treatment guide, statement from the attending physician attesting to the pregnancy or the need for the supplement, invoice/receipt for the purchase of the supplement, document issued by the Health Center where the beneficiary is registered, copy of the citizen card, document with the beneficiary's bank identification number (NIB).
